Year 9 & 10 Immunisation Information
Dear Parent/Guardian
Your young person is due 2 vaccinations that are free on the NHS and are given in year 9 as part of the national immunisation programme. The vaccinations are to protect against Diphtheria, Tetanus and Polio (3-in-1 Teenage Booster) and 4 strains of Meningitis (ACWY). For those children who are outstanding their Human Papilloma Virus, HPV vaccinations, for which a paper consent form has already been completed, this will also be offered at the appointment.
Meningitis ACWY vaccination helps to protect your child against four types of meningococcal bacteria (groups A, C, W and Y) that can cause meningitis (inflammation of the lining of the brain) and septicaemia (blood poisoning). These diseases are very serious and can kill, especially if not diagnosed early.
The 3 in 1 Teenage booster vaccination protects against Diphtheria, Tetanus and Polio, this should be the 5th dose which completes their childhood course; it boosts protection started earlier with baby and pre-school vaccinations against these diseases to give long lasting protection.
More information regarding these vaccinations can be accessed through the links below, please ensure that you read the information contained in the links below prior to booking an appointment and giving consent for your child’s immunisations. You must have parental responsibility or delegated authority to consent for the vaccinations.

Due to COVID-19 restrictions all vaccinations are being offered via an on-line booking system (fUSION), you will need to register to use this app, once this has been completed you can book your appointment.
Please ensure you know your child NHS number prior to booking, you can find this in your child’s “Red Book” or by using this link: https://www.nhs.uk/nhs-services/online-services/find-nhs-number/
Use the dropdown list and choose your child’s school, (please do not choose an alternative venue), you will then be able to choose an appointment slot, as your child will be in school they may not necessarily be asked to attend at the time you have booked. You will receive a confirmation email and text message with your appointment details on.
Please ensure only 1 parent / guardian books their child’s vaccination appointment to reduce the risk of duplicate appointments / vaccinations.
